Why did you show a picture of Alaska for the Louisiana sink hole?
Earthquake, 1985, offshore Acapulco. The energy just hammered Mexico City, but the real tragedy was the birth hospital built by the government for women that could not afford health care. If I remember correctly, some officials took kickbacks, & the cement for the facility was poured without regard. The seismic energy reduced the cement to powder as the hospital collapsed. I think (?) Over 280 patients died. Bad corruption. Very bad.
Don't forget Homestead, FL, that was leveled by Hurricane Andrew. Those houses were illegally built without required hurricane strengthening, definitely outside of code.
I would like to see one on the nasa space shuttle crashes. I would think that it would be expensive to replace a space shuttle
Ah, about the Chernobyl, the story if quite unexact… for instance, there nuclear chain reaction (nuclear explosion) didn't happen, it was "only" a power surge, the result of which was STEAM explosion…

In 2020, an orange man declared that an infectious disease was a hoax. The cost? 700,000 dead people due to his stupidity. And he walks free today. My neighbors were 2 of those who died due to his mistake.
Union Carbide.. Bhopal India. Perhaps it wasn't expensive as Union Carbide tried to blame it on employee sabotage. But more than 2000 died.
Putting a lock on a potentially dangerous piece of equipment during a maintenance shutdown (#5) doesn't hurt either ! ! !
Via-a-vis the other four fails could be categorized as engineering faux pas ! ! !
